Severe hypothermia with Osborn waves in diabetic ketoacidosis.

چکیده

Diabetic ketoacidosis causes substantial morbidity and mortality.1 Though the management of acute diabetic ketoacidosis centers on correcting hyperglycemia and electrolyte abnormalities, other important metabolic derangements may be seen. Specifically, hypothermia may be both a cause and consequence of diabetic ketoacidosis, and the severity of the hypothermia correlates with the severity of the illness. The case presented below highlights a presentation of severe diabetic ketoacidosis associated with hypothermia, which caused distinctive changes (Osborn waves) on the electrocardiogram (ECG). In the setting of diabetic ketoacidosis the recognition of ECG changes characteristic of hypothermia will help guide management and avoid the misdiagnosis of an ST-segment elevation myocardial infarction.

Recent findings suggest that hypothermia has conduction delay effects but the Osborn waves are direc...

Hypothermia: a complication of diabetic ketoacidosis.

During 1969-77, 20 episodes of severe hypothermia occurred in 19 diabetic patients in Nottingham. Thirteen were associated with ketotic hyperosmolar coma, two with lactic acidosis, and one with hypoglycaemia, while in four there was no loss of diabetic control. Ketoacidosis accounted for 11.8% of all admissions for severe accidental hypothermia and was a commoner cause than hypothyroidism (8%)....

Frequency of Diabetic Ketoacidosis and Severe Hypoglycemia in Children with Type 1 Diabetes

Background and Objective: Diabetic ketoacidosis and severe hypoglycemia are common and recurrent acute complications of type 1 diabetes and are associated with high mortality and morbidity. The aim of this study was to determine the frequency of diabetic ketoacidosis and hypoglycemia in children with type 1 diabetes. Methods: In this retrospective cross-sectional study, 480 patients with type 1...

Osborn waves of hypothermia.

A12-lead ECG was obtained from a 72-year-old man with a core body temperature of 85°F (Figure 1). The ECG shows severe sinus bradycardia with prolonged PR, prolonged QRS complex, prolonged QT interval, and an extra deflection at the end of the QRS complex (Osborn waves) (arrows). A second 12-lead ECG obtained 24 hours later, after the patient had been rewarmed to 98°F, was normal (Figure 2). Hy...

Objective: Patients with hypothermia are frequently admitted to the intensive care unit. Hypothermia may be associated with typical ECG changes (Osborn waves). Here, we present a typical case report with sequential ECG examinations from hypothermia to normothermia. Design: A case report. Patient: A 56-year-old woman was hospitalized with prolonged hypothermia after a suicide attempt with oxazep...
